How do I find a form?

The website provides forms and related documents for different activities. Use the Forms section to locate the document appropriate to your task.

Procedure

1. Open the Royal Arch Masons Ontario website.
2. Select Forms from the available website navigation.
3. Review the available form categories or listings.
4. Locate the form associated with your task.
5. Open or download the available form.
6. Read the form instructions before completing or submitting it.

Important notes

• Some forms may require an authorized account or apply only to particular offices or Chapters.
• Use the currently published form rather than an older saved copy when a newer approved version is available.
• Check any submission instructions, required signatures or supporting documents shown with the form.

Verification

Confirm that the form title and purpose match your task and that you know how and where the completed form must be submitted.
